Job summary
We are looking for a highly motivated and autonomous Practice Nurse to work 8 sessions per week (across 4 days). You will be supported by an established team of GPs, nurses, HCAs, and administrative staff, and play a vital role in delivering safe, efficient, and high-quality care to our patients.
Main duties of the job
Key Responsibilities
- Provide comprehensive clinical assessment, diagnosis, and management of patients presenting with both acute and chronic conditions.
- Undertake telephone and face-to-face consultations.
- Support the management of long-term health conditions.
- Contribute to QOF, enhanced services, and practice development initiatives.
- Work collaboratively with the wider clinical team and PCN to deliver patient-centered care.
- Assist with oversight and development of our current nursing team.
