Dementia Patients – Herbert Protocol

People with dementia can sometimes begin to wander — whether into the garden, the street, or further afield. While this may be brief, there are times when individuals get lost and go missing.

If you are a carer, family member or friend of a vulnerable person, you can fill in a Herbert Protocol Form. This form contains important details such as the person’s name, physical description, routine, and interests — helping the Police respond quickly if the individual goes missing.

The form can also be completed by someone living with early-stage dementia. You can download it directly from the Merseyside Police website (PDF).

Helpful Tips for Using the Herbert Protocol

  • Keep the form in a safe, easily accessible place in case the person goes missing.
  • Give a copy to trusted friends, family, or neighbours.
  • Update the information regularly whenever routines or health details change.
  • You only need to share the form with the Police if the person goes missing.
  • Don’t worry if you can’t complete every question — just provide as much as possible.
  • There is space at the end of the form to provide any additional information.
  • If printing, you can add extra pages; if emailing, include additional notes as needed.

Having a completed form can save crucial time and reduce stress if someone goes missing — allowing the Police to start searching more quickly.
