New Patients

We are currently accepting new patient registrations. 

All newly registered patients must complete a GMS1-New Patient Registration Form and a health questionnaire New Patient Health Questionnaire

We aim to complete your registration within 10 working days and all patients will receive a confirmation text message when registration is complete.  Medical treatment is available from the date of registration.

Please note we have an out of area registration policy. If you live outside the boundary which we are given by the ICB, we will recommend that you register with a GP in the area in which you live, to avoid any travel or emergency situations that may come up in the future.
