Home Visits

Home visits are provided for our bedbound patients.

All patients are encouraged to attend surgery where possible as this allows for better use of practice time and the clinician has full access to all equipment and patient medical notes.

West Wirral Group Practice has a wheelchair available to reception for patients to use.

All appointments are triaged through our online consultation system called patchs – PATCHS

If patients are unable to complete the form online they can call our reception team on 0151 909 2474 who will be happy to complete the form on the patients behalf.

All details are then triaged by the GP and a member of the team will then be in touch.

Please note that at this time some of our home visits are conducted by the our primary care trust,  Healthier West Wirral home visiting service.
