Birth Month Recall
Birth Month Recall System for Annual Health Reviews
We’ve introduced a birth month recall system to schedule annual reviews for patients with long-term (chronic) health conditions. You’ll be invited for a review during the month of your birth.
This system is designed to:
- Make it easier to remember your review date
- Ensure regular and consistent monitoring of your health
- Combine necessary checks (e.g. blood tests and medication reviews) where appropriate
Transition Period
Some patients may be invited earlier than usual during the first year to align with their birth month. We understand this may be frustrating if you’ve recently had a review, but we believe this change will be beneficial in the long term.
Conditions Included in the Birth Month Recall
The following conditions will follow the birth month recall system:
- Chronic Heart Disease (CHD)
- Chronic Kidney Disease (CKD)
- Diabetes
- Heart Failure
- Hypertension
- Peripheral Arterial Disease (PAD)
- Pre-diabetes (borderline diabetes)
- Stroke
- COPD
- Asthma
Patients with the following conditions will be recalled separately:
- Dementia
- Learning disabilities
- Mental health conditions
If you have more than one condition, you’ll only be contacted once per year — but multiple appointments may still be required.
How the Recall System Works
Invitation
You will be contacted around your birth month by text message or phone call.
Booking
Please call the practice to arrange your review appointment upon receiving your invitation.
Combined Reviews
Where possible, we aim to combine reviews and tests into a single visit to reduce inconvenience.
Flexibility
We understand that some patients may need flexibility. Please contact us if the proposed timing does not work for you.
Keep Us Updated
- If you’re invited for review but don’t believe you have one of the listed conditions, let us know. Occasionally, transferred records may contain outdated information.
- Please ensure your contact details are up to date so you don’t miss any reminders.
- If you do not wish to receive text messages, please inform us so we can update your preferences.
- Finally, if you have not had your review following your birth month, please get in touch so we can arrange this for you.
Declining a Review
You may choose to decline your annual review. However, a second invitation will be sent before you are removed from recall for the year.
We strongly encourage all patients to attend their annual review. These reviews are important for monitoring your condition, adjusting medications, and preventing complications.
Attending Your Appointment
You are welcome to bring a carer or support person with you to your appointment.
