Birth Month Recall System for Annual Health Reviews

We’ve introduced a birth month recall system to schedule annual reviews for patients with long-term (chronic) health conditions. You’ll be invited for a review during the month of your birth.

This system is designed to:

Some patients may be invited earlier than usual during the first year to align with their birth month. We understand this may be frustrating if you’ve recently had a review, but we believe this change will be beneficial in the long term.

If you have more than one condition, you’ll only be contacted once per year — but multiple appointments may still be required.


How the Recall System Works

Invitation

You will be contacted around your birth month by text message or phone call.

Booking

Please call the practice to arrange your review appointment upon receiving your invitation.

Combined Reviews

Where possible, we aim to combine reviews and tests into a single visit to reduce inconvenience.

Flexibility

We understand that some patients may need flexibility. Please contact us if the proposed timing does not work for you.


Keep Us Updated


Declining a Review

You may choose to decline your annual review. However, a second invitation will be sent before you are removed from recall for the year.

We strongly encourage all patients to attend their annual review. These reviews are important for monitoring your condition, adjusting medications, and preventing complications.


Attending Your Appointment

You are welcome to bring a carer or support person with you to your appointment.
